Honoring Heroes: Specialized Behavioral Health Care for Military Families

About Our Program
The West Oaks Hospital Patriot Support Program is a 36 bed Substance Abuse and Co-Occurring Dual Diagnosis Rehabilitation Program. Military Veterans are initially approved and authorized by the Veterans Administration for 30 days of Substance Abuse Rehabilitation. The program is voluntary and the actual number of days the Veteran remains in the program is individualized to meet each Veteran’s needs.
Program services are designed to meet the needs of Veterans with substance abuse issues, dual mental health disorders, and chronic medical disorders. The Multidisciplinary Treatment Team is led by a Psychiatrist Board Certified in Addiction Medicine. The Treatment Team consists of Licensed Chemical Dependency Counselors (LCDC), Licensed Clinical Social Workers (LCSW), Certified Trauma Resolution Therapists, Registered Nurses (RN), and Mental Health Technicians.
Medical Services are provided by the attending Psychiatrist who meets daily with each Veteran for Medication Management. Internal Medicine Physicians specializing in Addiction Medicine work with each Veteran for chronic medical conditions such as Hypertension, Diabetes, COPD, CAD, HIV, Cirrhosis, and other Internal Medicine needs. Nursing Services work with the Physician to meet the medical needs of all Veterans. Nursing Services dispense all medications, monitor the unit for safety issues, provide medication compliance education, and promote healthy living, eating, and sleep habits.
Clinical Services utilizes Evidence Based Practices such as Cognitive Behavioral Therapy (CBT) via Group, Individual, and Family Therapy Sessions. This includes Chemical Dependency Education, 12 Step Program Activities, Mental Health Education, Process Groups, Trauma Resolution, and Healthy Living Habits. Trauma Groups and Suicide Prevention groups are conducted weekly. Individual sessions for Eye Movement Desensitization and Reprocessing (EMDR) services are available for those Veteran’s with significant Traumatic Stress issues. Recreational Therapy services provide education for healthy living and non-chemical coping skill development.
Care Management Services
Care Management Services are provided individually to each Veteran by a dedicated Veteran Services Care Manager. These services include coordinating care with the Veterans Administration, Families, External Physicians, Government Agencies, Outside Organizations, and Sober Living Facilities. Passes are provided to Veteran’s when needed to attend urgent VA Medical appointments, visits to Sober Living Facilities, or other urgent matters. All additional needs that pertain to Veteran Services are managed by the Veteran Services Care Manager.
